Name matching: The legal entity name on the filing must exactly match the name on record with the OCC and FMCSA.

Duplicate filing risks: For changes, submit a new filing; cancellation (Form K) not required.

New applicant process: If the insured listed on the filing does not have an intrastate license in Oklahoma, the filing is held for up to 30 days before processing, in the event the insured is applying for a license. If the motor carrier does not apply for a license within 30 days, then the filing will be rejected.

Identification numbers: Oklahoma requires a PIN/Entity number. A DOT# is required if a PIN/Entity# is not available.
